Job Opening: Community Health Worker 3 (Lewiston)

MMHP is seeking a Community Health Worker 3 (CHW3) to support our mission to improve the health status of Maine’s seasonal workers and their families by providing culturally appropriate care and services. The CHW3 is directly responsible for facilitating access to care and services for Maine’s migrant and seasonal farmworkers (MSFWs) and their families.

For the past 25 years, the MMHP has provided health care access to migrant and seasonal farmworkers (MSFWs) throughout the state of Maine. A 501(c)(3) nonprofit organization and federally qualified health center (FQHC), MMHP has an innovative approach to medical care that is low-barrier, values cultural competency and embraces the human dignity of an invisible and vulnerable population. MMHP’s unique model brings medical, dental and behavioral health services to patients via mobile units where they live and work and by connecting clients to local partners.

The Maine Migrant Health Program is recruiting for a new CHW3 to provide health outreach services to MSFWs in Lewiston, Turner, and the surrounding area. Responsibilities to include: outreach, health education, health insurance assistance and care coordination, including interpretation. A high level of proficiency in both English and Spanish is required. Reliable transportation/clean driving record and basic computer skills are a must. The position is full-time at 32 hrs/wk, with some weekly administrative time in our office in Augusta, and will require a flexible daytime schedule, as well as some evening and weekend hours (varying seasonally).
